DESCRIPTION On the vehicles affected by this recall, confirm the emergency light sensors are well and truly installed in the appropriate orientation. PROCEDURE DANGER Park vehicle safely, apply parking brake, stop the engine. In the battery box, set the battery cut-off switch to the OFF position prior to working on the vehicle. Lockout & Tag out (LOTO) must be performed during set-up, maintenance or repair activities. Refer to your local procedure for detailed information regarding the control of hazardous energy. Confirm the axes on the sensor decal line up with the vehicle axes. The sensors just need to be mounted so that the X, Y, and Z axes on the decal line up with the vehicle axes. It does not matter if the X points to front or back of the bus, the top or bottom of the bus, or to either side of the bus. Just line up the sensor so that it lines up with one of the vehicle axes. DEFECT DESCRIPTION On certain vehicles, the emergency lights sensors could have been inadequately installed during the vehicle assembly. Consequently, the emergency lights could fail to activate after a rollover event. SAFETY RISK If the vehicle is involved in a rollover event, the emergency lights might not activate, thus potentially increasing the emergency passenger evacuation time and the risk of injury. Prevost has not received any reports of death or injury associated with this defect. Description of the Cause : During the vehicle manufacturing, certain emergency light sensors were installed in the incorrect orientation by Volvo assembly line technicians (installation error). The installation instructions specified the correct sensor orientation, but the instructions were not always followed in production. Identification of Any Warning that can Occur : There is no warning with this issue. The concerned vehicles are certain MY2023 Volvo 9700 buses manufactured during the indicated production dates. The recalled products are limited to these specific vehicles since they were the only ones equipped with the emergency lights option before the issue was solved in production. A total of 14 MY2023 Volvo 9700 vehicles are affected by the recall. Production Dates : MAY 01, 2022 – MAY 31, 2022 VIN Range 1 : Begin : NR End : NR Not sequential Description of Defect : Description of the Defect : On certain vehicles, the emergency lights sensors could have been inadequately installed during the vehicle assembly. Consequently, the emergency lights could fail to activate after a rollover event. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : If the vehicle is involved in a rollover event, the emergency lights might not activate, thus potentially increasing the emergency passenger evacuation time and the risk of injury. Prevost has not received any reports of death or injury associated with this defect. Description of the Cause : During the vehicle manufacturing, certain emergency light sensors were installed in the incorrect orientation by Volvo assembly line technicians (installation error). The installation instructions specified the correct sensor orientation, but the instructions were not always followed in production. Identification of Any Warning that can Occur : There is no warning with this issue. The incorrect sensor installation was identified by Volvo during an internal production inspection. 2022-09-09 Internal Volvo and Prevost investigation was opened to identify the root cause and to determine if the issue is safety related. 2022-11-01 Case was presented on Volvo Product Safety Working Group (PSWG) to evaluate if there is a potential safety related issue. 2022-11-10 Case was presented to Volvo Product Safety Committee (PSC). It was concluded that there was a safety related defect and that a recall is needed. 2022-11-15 Safety Recall report submitted to authorities. No other field reports, incidents, accidents, injury, or fatality were reported. Description of Remedy : Description of Remedy Program : Prevost will inspect and, if required, reinstall the emergency light sensors in the correct orientation on all potentially affected vehicles. Letters will be sent to customers to inform them on how to proceed. Prevost will execute this correction free of charge (parts and labor) for the customers. In case a customer has already inspected and corrected this defect before the safety recall has been launched, this customer will be reimbursed according to Prevost reimbursement plan. How Remedy Component Differs from Recalled Component : The issue concerns an inadequately installed component, and not a defective component. After the remedy, the emergency lights sensors will be adequately installed on the vehicle. Identify How/When Recall Condition was Corrected in Production : The recall condition was corrected in production on 2022-09-09 when the importance of following the emergency light sensor installation instructions was emphasized in production (additional training was provided to assembly line technicians).
